In this updated educational analysis, we have expanded our scope to over 350 generic formulations. We evaluate medications based on their Cost-Effectiveness Index (CEI), which measures “Hours of Duration per Rupee Spent.”
A high CEI indicates a substance that is both extremely cheap and stays in the body for a very long time—a combination that increases the risk of chronic accumulation and physical dependence.

Notable Findings in the Expanded List
The Fluoxetine Phenomenon (Rank #1)
With the inclusion of the full CNS portfolio, Fluoxetine emerges as the highest “value” drug. While it is an antidepressant (SSRI) and not a primary sedative, its active metabolite Norfluoxetine stays in the body for up to 16 days. At just ₹0.83 per dose, it has the lowest clearance rate per rupee of any drug in the Jan Aushadhi catalogue.
The Antipsychotic Value Index
Drugs like Risperidone and Quetiapine show moderate CEI values. While they are not classically “euphoric,” their sedating side effects are often exploited by those seeking cheap relief from insomnia or anxiety, especially given their low cost (₹0.94–₹1.03 per dose).
Benzodiazepine Consistency
The “Big Four” (Diazepam, Nitrazepam, Clonazepam, Alprazolam) remain in the top 10. Their high half-lives relative to their sub-rupee pricing make them the primary substances of concern for long-term dependence in the Indian market.
Conclusion: The Accumulation Risk
The CEI is more than just a number; it represents a toxicity risk. A drug with a high CEI is difficult to “wash out” of the system. If a user takes another dose before the first one has cleared (which, for Rank #1-5, takes weeks), the drug levels in the blood will eventually reach toxic or lethal concentrations.
Education on half-life is the most effective tool in preventing the “stacking” effect that leads to many unintentional overdose deaths in the community.
