Propranolol (widely sold in India as Ciplar) is biologically distinct from every other medication used in psychiatric care. It is a Beta-Blocker, originally designed to lower blood pressure. In Bipolar Disorder, it serves as the ultimate physical shield against medication-induced tremors and severe physical anxiety.
Section 1: What is this medicine & how it works
1. Drug Class & Category
- Primary Class: Non-Selective Beta-Adrenergic Blocker.
- Psychiatric Category: Anti-Tremor / Anxiolytic Adjunct.
2. Mechanism of Action (Simple Language)
When you are stressed or taking heavy psychiatric drugs (like Sodium Valproate or Lithium), your body releases adrenaline, making your hands shake and your heart race. Propranolol acts as a physical shield. It physically blocks adrenaline from attaching to your heart and muscle receptors. It slows the heart rate down and completely stops the physical shaking, though it does not cure the mental worry in your brain.
- Medical Detail: It competitively blocks Beta-1 receptors (located primarily in the heart) and Beta-2 receptors (located in the smooth muscle of the lungs and skeletal muscle). Blocking Beta-1 reduces heart rate and blood pressure. Blocking Beta-2 stops the skeletal muscle tremors.
3. Approved Medical Uses (Indications)
- Hypertension: High blood pressure.
- Angina Pectoris: Chest pain due to coronary atherosclerosis.
- Essential Tremor: Familial or hereditary shaking.
- Migraine Prophylaxis: Prevention of common migraine headaches.
4. Off-Label Uses
- Medication-Induced Tremors: The gold standard for treating the fine hand tremors caused by Sodium Valproate and Lithium.
- Performance/Somatic Anxiety: Used to stop the physical symptoms of panic attacks (racing heart, sweating, shaking voice) before stressful events.
- Akathisia: Used to treat the unbearable inner restlessness caused by antipsychotics like Olanzapine.
5. How Quickly it Starts Working
- Onset of Action: Immediate-release tablets work within 30 to 60 minutes.
6. Half-Life and Duration of Effect
- The biological half-life is 3 to 6 hours. This means immediate-release tablets must be taken multiple times a day for continuous tremor control. Extended-release (LA) capsules last for 24 hours.
Section 2: Dosage, Forms & How to Take It
7. Standard Adult Dose & Frequency
- For Medication-Induced Tremors or Anxiety: Typically 10 mg to 20 mg taken two to three times a day.
- For Migraine Prevention: Often higher, starting at 40 mg daily (usually using LA formulations).
8. Available Forms
- Standard Immediate-Release Tablets: 10mg, 20mg, 40mg.
- Long-Acting (LA/SR) Capsules: 40mg, 80mg (Designed to release slowly over 24 hours).
9. How & When to Take It
- With or Without Food: Propranolol is actually absorbed better when taken with food.
- Time of Day: Can be taken at any time. For performance anxiety, take it 45 to 60 minutes before the stressful event.
10. What to do if you miss a dose
- Take it as soon as you remember. If it is almost time for your next dose, skip it. Never double the dose, as this could drop your heart rate dangerously low.
11. Maximum Daily Dose
- For psychiatric use, doses rarely exceed 120 mg to 160 mg per day. (Cardiovascular uses sometimes go higher, but only under strict cardiology monitoring).
12. How to Store the Medicine
- Store at room temperature (below 30°C). Keep away from direct sunlight and moisture.
